Klamath County is recruiting for a Spanish speaking Services Coordinator to join the Developmental Disabilities Services Team. This position provides targeted case management services to individuals with Intellectual/Developmental Disabilities (I/DD). This role involves identifying individual needs and developing plans to access services and resources. The Ideal Candidate

Our Bilingual Services Coordinator is fluent in Spanish and has knowledge of the public service system for developmental disabilities services in Oregon. You should have a bachelor's degree in behavioral science, social science, or a related field, or equivalent experience in human services. You are detail-oriented, organized, and capable of managing multiple tasks. You excel in communication and are able to effectively work with individuals with I/DD, their families, and various stakeholders.

You have a proactive approach to problem-solving and a commitment to maintaining confidentiality and professional boundaries.

Key Responsibilities

* Develop and periodically revise an ISP or Annual Plan based on assessments, specifying desired outcomes, goals, and actions to address various needs.

* Assist individuals and families in developing and implementing a support system based on a Needs Assessment.

* Utilize choice advising to support individuals in selecting services, providers, and settings.

* Develop appropriate plans and monitor for effectiveness and Medicaid standards.

* Deliver services using person-centered practices and encourage self-determination.

* Authorize and deliver developmental disabilities services chosen by the individual or their representative.

* Coordinate with other state, public, and private agencies regarding services.

* Maintain knowledge of relevant laws, rules, policies, contracts, and regulations.

* Independently maintain a caseload of individuals in various service elements.

* Use tools like calendars and software to manage workload tasks.

* Document interactions with individuals through weekly progress note submission.

* Attend supervision sessions and utilize leadership for case consultation and training.

* Participate in transition planning by attending IEP and other meetings.

* Deliver case management services in compliance with OAR chapter 411, division 415.

* Report existing and insufficient service delivery resources.

* Enhance professional knowledge and complete a minimum of 20 hours of I/DD training per year.

* Develop and maintain relationships with community partners and stakeholders.

* Maintain confidentiality and act as a mandatory abuse reporter.

* Promote the mission and core values of KCDDS with professionalism and integrity.

Qualifications

* Education (must have one of the following):

* A bachelor's degree in behavioral science, social science, or a closely related field; or

* A bachelor's degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or

* An associate's degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing

* Certifications:

Valid Oregon Driver's License.

* Skills and Abilities:

* Fluency in Spanish (reading, writing, and speaking).

* Knowledge of the public service system for Developmental Disability Services in Oregon.

* Familiarity with Klamath County community agencies and resources.

* Understanding of mandatory reporting requirements and confidentiality rules.

* Proficiency in using Microsoft Office, record-keeping programs, and other relevant software.

* Ability to manage complex projects, collaborate effectively in a team environment, and maintain professional boundaries.

* Strong communication skills in English and Spanish, with effective presentation and public speaking abilities.

* Capacity to handle highly sensitive and confidential information appropriately.
