Infectious Disease Care Coordinator

Monday to Friday 8:00am – 5:00pm (no weekends, nights, or major holidays)

Full-time (40/hour per week)

—and want to make it easier for people to access life-saving infectious disease services? River Valley Health is seeking an Infectious Disease Care Coordinator to strengthen care coordination, outreach, and follow-up that improves engagement, retention, and health outcomes.

What you’ll do

You’ll coordinate and support infectious disease and related services across clinic and community settings, including:

  • Provide education and coordinate testing/care for PrEP/PEP
    , HIV treatment, viral hepatitis (HCV/HBV) testing & treatment, and STI services.
  • Collaborate closely with substance use disorder team members to support continuity of care.
  • Help patients navigate resources and funding options; coordinate pharmacy assistance programs, transportation, and interpretation.
  • Conduct focused patient engagement, follow-up, and outreach to close gaps in care and promote wellness.
  • Support patient understanding of treatment plans (prescriptions/refills, referrals, authorizations, when to seek care).
  • Participate in community events and provide health education;
    point-of-care HIV/HCV/STI testing training will be provided.
  • Complete internal data tracking/analysis and support grant reporting and quality improvement activities.

What we’re looking for

  • Experience working with and/or advocating for populations at risk for infectious diseases preferred
  • Prior management/supervisory experience in a healthcare setting strongly preferred
  • Excellent organization, communication, and relationship-building skills
  • Comfort with Word processing, EMR systems, and spreadsheets; data tracking/reporting experience strongly preferred
  • Willingness to travel regionally as needed
