Field Case Manager (RN)

Nuclear Care Partners is seeking an RN interested in Field Case Management along with clinical RN care. Candidates must be detail and deadline oriented, possess the ability to work closely with other non‑clinical and clinical team members, have a positive attitude, and be very reliable. The perfect candidate will be self‑motivated and have an excellent ability to provide leadership for the company, maintain a qualified staff that will meet patient needs, and successfully execute company initiatives.

Full time

• Patient Care
• Build relationships with patients and families
• Scheduling, preparation for and attendance of physician renewal/new patient appointments
• Initial nurse assessments and start of care visits
• Medication interaction checks
• Maintain patient binder in home
• Ensure implementation of company policies and procedures
• Ensure orientation, evaluation, and continuing education for all staff
• Responsible for overall supervision of all staff in the area

• Bachelor of Science in Nursing preferred
• Current RN licensure in the state of Tennessee
• Strong sense of professionalism, effective interpersonal skills, and true team player
• Previous case management or leadership role(s) including interviewing, hiring, onboarding, mentoring, and leading preferred
• Valid driver license and in good standing
• Must pass a pre‑employment screening
