Nurse Practitioner – In-Home & Telehealth Care

Position Overview

ACE Physicians is expanding into Tennessee, and we are looking for compassionate and motivated Nurse Practitioners to help bring high-quality, patient-centered care directly to the communities we serve.

This hybrid role provides care through in-home visits and telehealth
, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable. This is an exciting opportunity to join a growing organization and play a key role in establishing our presence in a new region.

We are seeking clinicians who are passionate about improving access to care, working with diverse patient populations, and delivering thoughtful, individualized treatment.

• Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments

• Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.
• Develop and implement individualized treatment plans tailored to each patient’s needs.
• Educate patients and families on health management, wellness, and disease prevention.
• Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care.
• Perform minor clinical procedures such as wound care as appropriate during home visits.
• Maintain accurate and timely documentation in electronic medical records (eClinical
  
  Works or similar platforms).
• Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities.

• Active, unrestricted Nurse Practitioner license (required).
• Experience in geriatrics, internal medicine, or primary care preferred.
• Previous home health or home-based care experience strongly preferred.
• Experience caring for patients with disabilities is highly desirable.
• Comfortable delivering care in both in-home and telehealth settings
