Covenant Health - - Responsibilities:
Coordinate and facilitate patient transfer and consult requests from referring facilities in accordance with clinical guidelines and EMTALA.;
Coordinate and facilitate direct patient admissions from physician offices and ambulatory centers.;
Coordinate and schedule tele-medicine consults for emergency departments and inpatient facilities.;
Obtain and document accurate patient clinical information and communicate to accepting physicians.;
Monitor bed status and coordinate bed placement across Covenant Health facilities.
