License Practical Nurse (LPN)- Overnights-Contract Partnership

Status:
Overnights- 8 and 12 hour shifts available.

Pay: $35.00 hourly pay + Shift Differential + Mileage

Position Summary:

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services, monitor client health, and ensure high-quality care according to individualized plans. This role collaborates with RN Case Managers, caregivers, and interdisciplinary teams to promote health, safety, and independence.

Key Responsibilities:

• Perform skilled nursing assessments and implement care plans under RN direction.
• Administer medications and treatments safely and accurately.
• Complete clinical documentation and report changes in client condition promptly.
• Communicate effectively with clients, families, and care teams to ensure continuity of care.

Qualifications:

• Graduateofan accredited LPN program.
• Active LPN license in good standing in Tennessee.
• Professional Liability Insurance required before start date
• Minimum 1 year of nursing experience preferred; home health experience a plus .
• Strong clinical judgment, documentation skills, and ability to work independently.
