This position is open to candidates located throughout the state of Tennessee.
Virtual:
The role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. Candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment unless an accommodation is granted as required by law.
The LTSS Service Coordinator-Clinician works under the direction/supervision of an RN, with overall responsibility for the member's case. The Clinician contributes to the LTSS care coordination process by performing activities within the scope of licensure, including assisting the responsible RN with telephonic or face-to‑face assessments and managing member needs, such as physical health, behavioral health, social services, and long‑term services and supports.

- Assist the responsible RN in identifying members for high‑risk complications and obtaining clinical data as directed.
- Assist in identifying members who would benefit from alternative levels of care or waiver programs.
- Provide all collected information to the responsible RN, who verifies, interprets, conducts additional assessments as necessary, and develops, monitors, evaluates, and revises the member’s care plan.
- Participate in coordinating care for members with chronic illnesses, co‑morbidities, and/or disabilities, working with the RN, member, and health‑care team to ensure cost‑effective utilization of health benefits.
- Make decisions based on current member needs, applying knowledge of disease processes, terminology, and clinical guidelines without requiring nursing judgment.
- Requires an LPN/LVN, LSW, LCSW, LMSW, or another license (other than RN) in accordance with applicable state law, a Nursing Diploma or AS in Nursing, or a related field; minimum of 2 years of experience working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role or an equivalent combination of education and experience.
- Current, unrestricted LPN/LVN, LSW, LCSW, or LMSW or another license (as allowed by state law) in the applicable state(s) required.
- Strong preference for case‑management experience with older adults or individuals with disabilities.
- BA/BS in Health or Nursing preferred.
