Private Duty LPN

Role: Home and Community Based Services Licensed Practical Nurse, providing direct nursing care in patients' homes under supervision.

• Deliver high-quality clinical services within legal scope, following care plans and infection control procedures.
• Record thorough documentation of patient visits in electronic medical records and visit verification programs.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
• Adapt to various home environments, ensuring equitable care regardless of patient background.

• Unrestricted LPN licensure in the practice state.
• Valid driver's license, vehicle insurance, and reliable transportation or access to public transit.
• Preferred: CPR certification, flexibility, multitasking ability, independence, and strong communication and organizational skills.

Provide clinical care, documentation, and care coordination to ensure quality patient outcomes in a home setting.
