Amedisys is seeking a Nurse Practitioner to provide hospice and home health care in Tennessee. You will work with clinical teams to assess patients, manage symptoms, and coordinate care in patients’ homes.
Responsibilities include conducting face-to-face encounters, ordering labs and imaging as needed, and participating in interdisciplinary team conferences. Requires NP licensure, Medicare billing privileges, and CPR certification; prior hospice/home health experience is preferred.
