Gentiva Hospice is seeking an experienced RN Case Manager to provide in-home hospice nursing, assess patient needs, and develop personalized care plans. You will work closely with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, high‑quality end‑of‑life care.
You will educate patients and families, coordinate visits, and maintain precise clinical documentation while upholding regulatory standards. A TN RN license and hospice/home health experience are preferred.
