Case Manager I
Location: Knoxville, TN
Status: Full Time, Hourly, Non‑Exempt
Program: VOA Honor
Reports To: Program Director
Job SummaryThe SSVF program assists veterans who are homeless or at risk of homelessness in ending their crisis. The Case Manager will provide supportive services, education, and resource linkages to empower veterans to maintain long‑term housing stability and self‑sufficiency.
Qualifications- Bachelor’s degree in social work or related field; or
Preferred if at least five (5) years of experience in the field, or 3 years of related experience for a veteran. - Demonstrated ability in family needs assessment, resource access, advocacy, counseling, and collaboration with local service providers.
- Excellent verbal and written communication, strong organization, and creative problem‑solving.
- Valid driver’s license, automobile, liability insurance, with up to 70% travel between counties.
- Will complete required case‑management and Critical Time Intervention (CTI) training within 90 days of hire.

- Provide outreach to increase awareness of SSVF and recruit veteran families.
- Conduct assessments and develop/monitor case plans.
- Establish linkages with agencies and service providers.
- Coordinate with VAMCs and other providers for health and daily living services.
- Allocate resources based on participant need.
- Educate participants on available services and rights.
- Assist in obtaining VA benefits and other public benefits.
- Provide referrals for financial planning, transportation, income support, fiduciary/representative payee, legal services, childcare, and housing counseling.
- Apply clinical judgment in decision making.
- Relate to veterans and families culturally appropriately.
- Participate in PQI duties as assigned by supervision & PQI Committee.
- Facilitate Critical Time Intervention (CTI) model with program participants.
- Collaborate with SSVF case managers, intake coordinators, and outreach workers; attend regular case consultations.
- Advocate for veterans, integrating their needs into case plans.
- Serve as liaison with local HUD‑VASH teams and attend case conferences.
- Address barriers faced by clients in urban and rural areas.
- Comply with all program policies and Council on Accreditation procedures.
