About the Position & Its ImpactThe Alumni & Continuing Care Coach is responsible for guiding clients through their transition out of Onsite programs, helping them build and sustain meaningful aftercare plans while maintaining strong relationships with referral partners.
This role is where transformation is protected and extended. Healing doesn’t end when a client leaves a program—it evolves in the real world. You will walk alongside clients in that critical re-entry phase, helping them stay connected, supported, and grounded in their progress. Your work ensures that clients don’t just experience breakthrough—but continue living it, with the right resources, relationships, and encouragement surrounding them.
About the Day-to-Day in This Role- Provide ongoing coaching and support to clients as they transition back into daily life, helping them navigate re-entry with clarity and confidence
- Partner closely with clinical teams to develop thoughtful, individualized continuing care and aftercare plans
- Maintain consistent communication with clients during and after their program to ensure continuity of care and support
- Build and nurture relationships with referral partners and providers to create a strong, trusted network of ongoing care resources
- Coordinate therapist referrals and community-based support aligned with each client’s unique needs and goals
- Track and evaluate client satisfaction and the effectiveness of aftercare plans, continuously improving the experience
- Maintain and grow a robust referral database to ensure clients have access to high-quality, aligned resources
- Provide follow-up support post-discharge, helping clients stay connected and reducing risk of relapse or disconnection
- Collaborate with marketing and business development teams to stay aligned on referral relationships and opportunities
- Contribute to a culture rooted in Onsite’s ANCHOR values, helping create an environment of authenticity, care, and connection
- Support reporting and insights related to continuing care outcomes and opportunities for growth
