This position is three days a week:
Monday, Wednesday and Friday.

Responsibilities
  • Provide prescribed medical treatment and personal care services to patients with diseases and injuries, as directed by a physician or mid-level provider.
  • Collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.
  • Assist with patient care, physical examinations and ancillary tests.
  • Perform annual and routine nursing assessments and preventive health services authorized by the VA and the clinic Medical Director, including interviewing and collecting patient information (pain assessment, vital signs, height/weight, etc.). Record information in the patient’s electronic medical record.
  • Administer medications as ordered by a medical provider (orally, subcutaneous or intramuscular), document timing and dosage in the patient’s electronic chart, apply compresses (hot and cold) as indicated, and observe for adverse reactions.
  • Operate electrocardiograph (EKG) and other diagnostic equipment to administer routine diagnostic tests and treatments (e.g., glucometers, pulse oximeters, nebulizers, oxygen equipment, spirometry, eye exams).
  • Dress wounds and incisions, interpret physician instructions to patients, assist with emergencies, and perform related tasks. Collect lab specimens (e.g., urine, blood, sputum) and may perform waived laboratory tests.
  • Maintain treatment rooms, equipment and supplies using appropriate cleaning methods.
  • Submit laboratory and radiology results to medical providers and provide results to VA for scanning into the patient’s medical record after provider review.
  • Input patient information into VISTA/CPRS and complete clinical reminders due at each visit.
  • May assist with front desk duties for check-in, check-out and phone coverage; support co-managed care collaboration with non-VA providers and facilities.
  • Provide patient and family health education with a focus on self-management, prevention, wellness, related health issues and healthy lifestyle choices; advise on services available through community resources.
  • Maintain confidentiality of veterans’ data and information.
  • Comply with VA and company training requirements.
Compliance
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.
Clinical Outcomes
  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
Customer Service
  • Provide excellent customer service to veterans, both in person and over the phone, as well as to colleagues and clinic visitors.
  • Establish and maintain a warm, welcoming and professional atmosphere for colleagues and veterans, ensuring needs and expectations are consistently met.
  • Collaborate with Clinic leadership to resolve veteran concerns or complaints.
Enrollment Growth
  • Participate in the clinic’s outreach events to support enrollment initiatives.
  • Support patient enrollment and retention in appointments, follow-up visits or nurse visits.
Requirements
  • Diploma or certificate of an accredited practical or vocational nursing program as approved by the state.
