Home Health Physical Therapist – PRN

• Plan and administer prescribed physical therapy treatment for patients in their homes
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals
• Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient-team goals
• Provide PTAs and Home Health Aides with written care instructions and oversee their performance and documentation
• Promptly and thoroughly document patient observations, interventions, and evaluations
• Report patient progress to physicians, Clinical Manager, care team, patients, and families
• Submit required evaluations, plans, progress notes, and discharge summaries

• Current and unrestricted Physical Therapy license
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Valid state driver's license
• Personal vehicle liability insurance meeting residing-state minimum limits or specified higher limits
• One year of experience as a Physical Therapist (preferred)

Demonstrates expertise in planning and administering physical therapy treatments, developing rehabilitation programs, and collaborating with healthcare teams. Proficient in patient assessment, documentation, and communication with care teams and families.
