Social Worker will be Lic. MSW in Texas, with at least 3+ years of experience. Social Worker will be responsible for:

• Performing discharge assessments on the floor and in the Emergency Department
• Identifying High‑Risk patients and assessing their potential needs
• Working with a multi‑disciplinary team within and outside the hospital to secure services and referrals for patients
• Following up with transition patients who may readmit to the hospital and completing a discharge assessment, including a readmission assessment
• Communicating daily via email, phone, or in person with the Transitions of Care NP and following up with patients who may be in‑house or in the emergency department
• In addition to seeing transition patients, this Community Connections Social Worker (CC‑SW) will see patients in the Emergency Department daily that have been identified as in need of service. This can be a referral from the physician, nurse, UR nurse, or the patient and/or patient’s family or the CC‑SW own review of records.

MUST HAVE GENERAL ACUTE CARE HOSPITAL EXPERIENCE. IT IS A MUST.

• Minimum of 2 years’ experience in an acute care or community health setting.
• Strong leadership and excellent interpersonal skills to effectively coordinate and coach others to deliver care in a coordinated, efficient and effective manner.
• Current SW certificate.
