The Home Health LVN Clinical Coordinator is responsible for supporting the coordination and oversight of patient care services under the supervision of a Registered Nurse (RN) or Clinical Manager. This role involves assisting with clinical supervision, ensuring compliance with care plans, and facilitating communication among the interdisciplinary team. The LVN Clinical Coordinator plays a key role in maintaining quality standards and continuity of care for patients receiving home health services.
Essential Functions:
Assist in coordinating patient care services and staff assignments.
Monitor patient condition and report changes to RN and physician.
Support compliance with agency policies, procedures, and regulatory requirements.
Document care coordination activities accurately and timely.
Participate in interdisciplinary team meetings and case conferences.
Provide patient and caregiver education as directed.
Promote infection control and safety practices during patient care.
Support quality improvement initiatives and staff training.
Assist with staff competency assessments and evaluations.
Respond to PRN emergency calls and participate in on-call rotation as assigned.
Job Qualifications:
Education:
Graduate of an accredited vocational nursing program.
Licensure:
Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license.
Experience:
Minimum two years of clinical experience; one year of home health experience preferred. Management experience preferred.
Skills:
Strong clinical skills and understanding of home health care standards.
Effective communication and interpersonal skills.
Ability to work independently and manage multiple priorities.
Proficiency in documentation and use of EMR systems.
Environmental and
Working Conditions:
Works in patients’ homes in various conditions, possible exposure to blood, bodily fluids, and infectious diseases.
Requires ability to travel locally and work a flexible schedule.
May be exposed to unpleasant weather conditions.
Physical and Mental Effort:
Requires prolonged standing and walking, ability to lift up to 50 pounds, and move patients.
Must be able to manage stressful situations calmly and courteously and make quick decisions to meet patient needs.
