Overview:
The Hospice RN Case Manager is responsible for coordinating and providing high-quality hospice care to patients. This role involves developing individualized care plans, managing patient visits, collaborating with the interdisciplinary team, and ensuring compliance with hospice standards and regulations. The RN Case Manager serves as the primary point of contact for patients and families, providing compassionate support and clinical expertise throughout the hospice journey.
Essential Functions:
Conduct comprehensive assessments of hospice patients.
Develop and implement individualized care plans.
Coordinate care with physicians, social workers, chaplains, and other team members.
Provide skilled nursing care and symptom management.
Educate patients and families on disease progression and care expectations.
Document all patient interactions and care provided in EMR system.
Participate in interdisciplinary team meetings and care planning.
Ensure compliance with hospice policies and regulatory requirements.
Respond to patient and family needs in a timely and compassionate manner.
Job Qualifications:
Education:
Graduate of an accredited school of nursing.
Current RN license in the state of practice.
Hospice and Palliative Care certification is preferred.
Experience:
Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care preferred.
Skills:
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal abilities.
Proficiency with EMR systems and documentation.
Ability to work independently and manage a caseload.
Compassionate approach to patient and family care.
Environmental and
Working Conditions:
Works in patient homes and hospice facilities.
Requires local travel to patient locations.
May be exposed to varying home environments and weather conditions.
Physical and Mental Effort:
Requires prolonged periods of driving and patient care.
Must be able to manage stressful situations with empathy and professionalism.
Requires working under some stressful conditions to meet patient needs and regulatory deadlines.
