• Full Time: 1st Shift
• $21 - $22 / Hour (Based on Experience)
• MUST be a Certified CNA

• Schedule and/or provide transportation for residents to and from appointments
• Assist residents safely during transport and appointments
• Provide resident care according to an approved Care Plan
• Assist with daily activities, personal care, and resident safety
• Maintain and respond to resident medical record requests
• Follow infection control procedures and safety protocols
• Complete required documentation, including vitals
• Report any violations of the Residents’ Bill of Rights immediately

• Completed CNA course and active state license
• Must be 25 years of age or older
• Clean driving record
• High school diploma or GED
• Medical Records experience in a long‑term care setting (preferred)
• ART certification (preferred)
• Strong written and verbal communication skills
• Service‑oriented mindset with a willingness to learn
• Positive, can‑do attitude and excellent attendance
