RN, Registered Nurse | Cardiology & Congestive Heart Failure Clinic

Registered Nurse – Cardiology & Congestive Heart Failure Clinic, Emplify Health by Gundersen, La Crosse, WI.

• 0.8 FTE (64 hours per pay period)
• 8‑hour day shifts: 7:30 AM – 5:00 PM
• No weekends, holidays, or on‑call requirements
• Primary location:
  Main Clinic in La Crosse; possible hours at Onalaska Clinic

• Perform a wide range of ambulatory cardiology nursing functions, including:
  • Phone triage and patient education (including cardiac procedures)
  • Medication refills and optimization visits
  • Managing Epic In‑Basket and MyChart patient messages
  • Coordinating cardiac procedures
• Support and manage patients in the Congestive Heart Failure Clinic, including:
  • Caring for a panel of heart‑failure patients
  • Conducting nurse‑led visits focused on medication optimization and assessment
  • Building long‑term, impactful relationships with patients
• Collaborate with Community Paramedics, Bellin Health, and inpatient teams to coordinate comprehensive care

• Associate Degree in Nursing (minimum requirement)
• Active RN license to work in Wisconsin
• Basic Life Support (BLS) certification for healthcare providers
• Minimum of 1 year of RN experience (Cardiology and/or ambulatory experience preferred; General Medical experience acceptable)
