Case Manager - Justice Program

Provides professional case management, assessment, and service coordination for individuals involved in justice-system programming.

• Interviews and assesses program participants using various evidence-based risk and needs assessment tools and reports assessment results and findings to District Attorney’s Office attorneys.
• Performs case management duties, including monitoring participant progress toward completion of court-ordered conditions, meeting with participants on a regular basis, referring participants back to Court for revocation as appropriate, and assisting participants with the program discharge process.
• Assists participants with addressing needs by referring to appropriate resources, tracking service connections, and monitoring follow-through with appointments and services.
