Case Manager

Humana is seeking a Case Manager for the La Crosse/Sparta area to coordinate whole-person care for members across clinical, behavioral, and social needs. In this role, you will manage a caseload of diverse members, conduct assessments, and create individualized care plans that promote health, independence, and quality of life. You will connect members to providers, benefits, and community resources, monitor progress, and adjust plans as needs evolve.

Working in a collaborative, member-focused culture, you'll partner with clinicians, social services, and families while leveraging data and digital tools to drive better health outcomes.
