Schedule: Full-Time, 40 hours per week

LSS is seeking a compassionate and organized Service Facilitator to partner with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate services that promote independence, well-being, and growth.
Compensation based on education and/or credentials:
Master's Degree: $27.20/hr
Master's w/therapy licensure: $28/20/hr
What You'll Do- Conduct functional and needs assessments.
- Develop and implement person-centered service plans.
- Coordinate services and connect clients with community resources.
- Facilitate team meetings and collaborate with families, providers, and support networks.
- Maintain timely, accurate documentation and client records.
- Advocate for clients and support positive outcomes through trauma-informed, strengths-based practices.
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field.
- Experience working with individuals, families, or vulnerable populations preferred.
- Strong communication, organization, and relationship-building skills.
- Ability to manage documentation, coordinate multiple services, and work independently.
- Valid driver's license, reliable transportation, and ability to travel within the community.
