• Assessment of referred patients requiring rehabilitation services.
• Provide direct patient care, determine functional capabilities, limitations, and potential for rehabilitation.
• Establish and administer treatment programs with goals set in collaboration with the physician.
• Document and record patient conditions and educate patients and families on physical therapy methods.
• Develop a plan of care for each patient and provide skilled interventions per physician orders.
• Assess patient needs, evaluate and modify care to meet physical therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

• Doctorate in Physical Therapy or Bachelor’s in Physical Therapy with passed NPTE.
• Graduated from an accredited PT program.
• Minimum one year of clinical experience.
• Current or pending state license as a Registered Physical Therapist.
• Current CPR certification.
• Other client‑specific requirements as determined by the facility.
