Pride Health is hiring a Social Worker tosupport our client's healthcarefacility in La Grande, OR.
Position OverviewWe are seeking a compassionate and experienced Social Worker .to provide psychosocial support and care coordination for patients in their homes throughout the La Grande, Oregon service area. This role involves conducting assessments, connecting patients with community resources, providing counseling and emotional support, and collaborating with the interdisciplinary care team to ensure high-quality, patient-centered care.

Pay Range: $35/hr - $40/hr
* Pay offered is based on experience,expertise, credentialing, and education.
Duties- Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, financial, and environmental needs.
- Develop individualized care plans and provide counseling and emotional support to patients and their families.
- Connect patients with community resources, financial assistance programs, transportation, and other support services.
- Education: Master's Degree in Social Work (MSW)
- Certification(s): Eligible for or active Oregon Social Work licensure
