Position Overview
We are seeking a dedicated Registered Nurse to provide high-quality home health care to patients across multiple counties. This role focuses on delivering skilled nursing services, completing OASIS assessments, and ensuring comprehensive patient care in a home setting. The ideal candidate brings strong clinical skills, attention to detail, and an ability to work independently in a field-based environment.
Schedule & Work DetailsLocation:
Coverage across Jefferson, Oldham, Henry, Trimble, and Carroll counties
Shift: Monday through Friday, 8:00 AM to 4:30 PM

Hours:
32-hour guarantee
On-Call/Weekend Requirements:
One weekend per month and one weekday per week
Mileage Reimbursement:
Yes, reimbursed between patient visits from first to last patient
Pay Rate: $51 to $53 per hour
Key Responsibilities- Provide skilled nursing care to patients in a home health setting
- Complete accurate and timely OASIS assessments and documentation
- Monitor patient conditions and update care plans as needed
- Educate patients and families on disease management, medications, and care plans
- Collaborate with interdisciplinary care team members to ensure continuity of care
- Maintain compliance with agency policies, regulatory requirements, and best practices
- Manage a caseload efficiently while maintaining quality and patient satisfaction
- Active Registered Nurse (RN) license
- Minimum of 1 year of nursing experience
- Ability to work independently in a field-based home health setting
- Strong clinical assessment and documentation skills
- Homecare Homebase (HCHB) experience
- Prior home health nursing experience
- OASIS documentation experience
- Strong hands‑on patient care background and time management skills
