SUMMARYThe Early Intervention Coordinator is primarily responsible for the facilitation of child and adult-centered services and supports. This position requires the coordination of service delivery and identification of resources in response to family or individual needs. The Coordinator will provide Service Coordination services in various areas, including EI, Part C Services, Children’s Extensive Support (CES), Home Community Based Programs for the Developmentally Disabled (HCBS-DD), Supported Living Services, both Waiver and State (SLS), and Service Coordination Only.
In addition, the Coordinator will provide direct monitoring of contracted professionals, allocate resources to assigned programs, and work toward the goals of the individuals in service and the agency within a person-centered environment.
Responsible for the facilitation of receiving services and supports for family/persons applying for service, as well as termination and/or transfer of services within established timelines.
Coordinate initial and all subsequent Individual Family Service Plan meetings to develop a plan based upon individual needs, implementing and monitoring services and supports. Ensure schedules of service changes are completed, submitted as necessary, and expenditures are within guidelines established by the State of Colorado.
Perform billing functions for children in service in accordance with established agency, state, and federal rules and regulations within established timelines.
Maintain regular contact with children and adults in service to assess satisfaction, advocate on behalf of the individual to ensure access to appropriate services, and function as a liaison between consumers, families, guardians, program staff, other agencies, and providers.
Ensure appropriate maintenance and completeness of master files, including periodic purging and/or destruction of records according to agency policies and procedures.
Pass the Medication Administration test with no more than two attempts and administer medication to individuals per physician orders using appropriate techniques.
Perform lifting and transferring techniques as trained; lift/push/pull up to 50 lbs as required.
Drive agency vehicles as necessary for transportation of individuals and agency business.
Meet physical and behavioral requirements, including ability to sit, walk, stand, climb stairs, restrain individuals when trained, and sit at a computer for extended periods.
Communicate with individuals and the public in both quiet and noisy environments; operate a personal computer and related software; tolerate stress and meet deadlines.
Work and concentrate in both noisy and quiet environments, squat, stoop, kneel, reach above head, and perform forward motions as needed. Work inside the building majority of the time.
ADDITIONAL DUTIES AND RESPONSIBILITIESObtain and maintain program accreditation through Early Intervention and the Colorado Department of Health Care Policy and Financing and other regulatory agencies.
Responsible for prior-authorization of Medicaid waiver services within established timelines.
Investigate allegations of Abuse/Neglect/Mistreatment or exploitation of individuals receiving services when assigned.
Recruit, set up, assist, monitor, and maintain contract providers and resources to connect families and individuals with community services, ensuring providers receive appropriate training.
Maintain program databases, input vital information, and compile reports as required by agency, state, and federal regulations.
Provide information and events that promote community-wide programs, facilitate training opportunities, and maintain a record of needs.
Participate in scheduled Service Coordination meetings and training sessions to stay informed on issues affecting case coordination.
Plan and participate in agency events and staff training activities per the staff development curriculum.
