Clinical Nurse BU – Hospice Skilled Nursing – La Mesa – Full‑time – Day Shift

Hourly Pay Range: $56.580 – $70.170 – $83.760
The pay scale is defined by the collective bargaining agreement between Sharp Health Care and the union representatives; placement is based on RN experience.

What You Will Do

Provide direct and indirect nursing services to patients and families.

Required Qualifications
  • Graduate of nursing school.
  • 1 year clinical experience.
  • California Registered Nurse (RN) – CA Board of Registered Nursing – REQUIRED.
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) – American Heart Association – REQUIRED.
  • Driver's license – CA Department of Motor Vehicles – REQUIRED.
Preferred Qualifications
  • HPCC Certified Hospice and Palliative Nurse (CHPN) – Hospice and Palliative Credentialing Center – PREFERRED.
Other Qualification Requirements
  • Utilize reliable transportation and possess adequate personal insurance coverage. Demonstrate a clean driving record in accordance with the employer DMV pull notice program and Sharp Health Care Driver Guidelines.
Essential Functions
  • Clinical advancement – Use systematic planning, appropriate methods, and timely exchange of information with the multidisciplinary team and patient/family to coordinate care. Demonstrate ability to focus on and manage multiple patient needs and aspects of care. Actively communicate with the care team to plan patient care. Utilize available resources to problem‑solve and determine the most effective approach to action.
  • Clinical judgment and decision making – Utilize the nursing process and a critical‑thinking approach to provide patient care that incorporates mind, body and spirit; is individualized, goal‑directed, and consistent with current standards of safety. Identify actual or potential problems for which the patient is tinuously evaluate interventions and care, and adjust the plan as patient and family needs change.
  • Collaborate with patient, family, and other team members to develop a comprehensive plan of care. Prioritize demands for resources and collaborate with others to meet individualized patient needs. Act as preceptor/teacher in guiding other nurses on how to apply the nursing process with the unit population.
  • Initiate appropriate referrals based on the information obtained in initial and ongoing assessments and evaluation. Implement safe, therapeutic and efficient care for patients with complex needs due to multi‑system disease and complications of treatment.
  • Document per department/entity guidelines of care and policies and procedures. Provide comprehensive, detailed, concise documentation. Serve as a resource for other staff in managing unit‑specific documentation issues.
  • Clinical leadership – Delegate, assign, and communicate expectations for care delivery to each team member. Know the limits of delegation for different health‑care team members. Ensure delegated tasks are completed and follow through. Assess appropriateness of physician orders and question physicians when appropriate.
  • Support coworkers by providing assistance, positive feedback, and constructive insight while maintaining a non‑judgmental, confidential approach. Seek opportunities to assist and improve collaborative work processes and systems.
  • Nurse‑patient/family relationship – Establish a therapeutic relationship with patient and family. Anticipate and address barriers such as spiritual, cultural, language or other communication challenges. Serve as a patient advocate by removing obstacles and supporting patient and family needs. Provide age‑appropriate education on recovery, self‑care, end‑of‑life care, health maintenance, and wellness. Coordinate transfer and discharge planning and teach as part of a multidisciplinary team.
