Position Overview
Service Coordination, Inc. (SCI) is seeking Supports Planners to provide case management services and support to people with disabilities and other complex medical needs within their communities.
Key Responsibilities- Advocate on behalf of people served, provide education and resources to promote self‑determination.
- Collect eligibility information, obtain documentation, and develop Person‑Centered Plans of Service (POS).
- Facilitate the person‑centered planning process, write and update POS documents in collaboration with clients.
- Research, refer and coordinate community resources and services to achieve POS outcomes.
- Monitor POS progress, conduct on‑site visits, collect data, and report events as required.
- Maintain HIPAA‑compliant records, track time, and adhere to SCI policies and performance benchmarks.
- Education:
Bachelor’s or Master’s degree in Human Services Administration, Psychology, Sociology, Social Work, Nursing, or Counseling. - Valid driver’s license and a reliable vehicle.
- Preferred experience working with people with disabilities and with person‑centered planning.
- Knowledge of Medicaid Home and Community‑Based Services and state‑mandated Supports Planning requirements.
- Physical ability to remain largely stationary, operate a computer, and occasionally move office equipment.
- Problem solving, decision making, initiative, trustworthiness, respect for others, interpersonal communication, relationship building, service orientation, teamwork, dependability, flexibility, detail orientation, professionalism, and planning.
- Competitive base salary: $64,395–$79,861, commensurate with education and experience.
- Annual quarterly bonus potential.
