MyCare Medical is actively seeking a full‑time, certified Nurse Practitioner/Physician Assistant to join our team in Mission, TX. We are a physician‑founded, dyad‑led medical practice specializing in primary care. Our commitment is to deliver the highest quality care to patients at the lowest possible costs, while providing a structural support system that allows providers to focus solely on patient care.

- Competitive salary and annual quality bonus – base salary range $125,000–$145,000, based on experience, with up to 15% of base salary as performance bonus

- Full support both in‑clinic and through our Central Business Office to ensure providers can focus solely on patient care
- Monday – Friday, 8 a.m.–5 p.m.
- Rotating and flexible on‑call schedule
- Float between Baytown and LaPorte with an emphasis on LaPorte. We use eClinical Works (eCW) and partner programs to streamline patient and practice workflows
- Focus on continuity of care, health maintenance, and disease prevention, working with patients and their families to diagnose, treat, and manage a population with multiple comorbidities
- Integrated team‑based care: collaborate as a multidisciplinary team to improve patient outcomes by meeting patient needs and preferences
- Prioritize preventative care and proactive management of chronic conditions to improve outcomes, reduce patient costs, and increase practice revenue
- Providers must be comfortable managing a primarily adult‑aged patient population
- Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC), focusing on adult medicine
- Certification through an accredited PA certification program focusing on adult medicine
- Licensed as an Acute, Adult/Gerontological, or Family Nurse Practitioner in the relevant state, or ability to obtain a license prior to employment
- Preferred candidate: minimum of two years’ experience as a Nurse Practitioner
- State‑eligible for DEA and CDS licensure, if required
- Up‑to‑date CPR and Basic Cardiac Life Support certification
- Strong organizational and prioritization skills
- Proficiency with Electronic Medical Record (EMR) systems
- Experience with Medical Advantage, Primary Care – FFS, Value Based Care, HEIDS, MRA, and ICD‑10 Coding
- Demonstrated ability to problem‑solve complex and multifaceted situations
