GLOBAL HOME HOSPICE SERVICE INC is looking for a Per Diem Licensed Vocational Nurse for Continuous Care Shifts, a member of the home care team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the client’s needs. The team is looking for a dedicated individual with a passion for care and values honesty and commitment.

The candidate must be willing to work 8 to 12 hour shifts when needed.

• Provides direct patient care as defined in the California State Nurse Practice Act
• Manages a patient’s symptoms during a short-term medical crisis
• Recognizes and interprets symptoms, institutes remedial measures, and reports significant changes in the patient condition immediately
• Charts the patient’s condition hourly to provide accurate and timely documentation of patient services to reflect the plan of care
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Assesses and provides patient and family/caregiver education and information pertaining to diagnosis and self-management of care
• Uses equipment and supplies effectively and efficiently
• Provides appropriate pain/symptom management and evaluates patient’s response to treatments/medications
• Participates in personal and professional growth and development; also participates in quality assessment, performance improvement teams, and activities
• Performs other duties as assigned by the registered nurse
• Is able to travel to other areas
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
• Calls the Medical Director if there is a new order for medications, writes the doctor's order, and orders the medication from the Pharmacy

• Graduated from an accredited Vocational Nursing Program
• Hospice experience preferred
• Currently licensed as a LVN in the State of California
• Demonstrates good verbal and written communication and organizational skills
• CPR certification
• California Driver's License
• Auto Insurance
• Understands the philosophy of the hospice concept and the needs of the terminally ill
• Physical Exam, TB Test or Chest X-ray
• Professional Liability Insurance

• Job Type: Part-time, Contract
• Pay: $30.00 - $40.00 per hour
