Under direction of the Program Manager, the Family Services Advocate identifies health and social service needs of assigned families and children as well as plan and implement program activities to meet those needs.

The Family Services Advocate uses a case management approach, they supports families in making choices and decisions that reflect and respond to the unique culture, role, values strengths and beliefs of the family and their relationship to the program by enhancing the conditions and quality of life of all family members.

• Initiate collaborative partnerships with parents to establish a rapport and encourage self-sufficiency.
• Assess each family's need for social services; develop an individualized plan that responds to the needs of the family; deliver and/or coordinate the delivery of needed social services to each family; review and update as required.
• Develop Individualized Family Partnership Agreements (IFPA) that describe the family's goals, strengths, responsibilities, strategies for achieving these goals, and timetables; build upon any pre-existing family plans; review and revisit with families so the IFPA remains current and useful.
• Provide comprehensive community resources information to families, individualizing to respond to the family's needs and concerns to ensure identified needs are met.
• Refer families to community agencies/programs; assess accessibility, relevance and usefulness of assistance received.
• Assist with the establishment and maintenance of ongoing collaborative relationships with community organizations.
• Conduct home visits with families as needed.
• Incorporate and encourage parent volunteerism throughout the program.
• Assist parents with the process of transition from home to school, program to program, and school to school or another setting.
• Prepare and enable parents to become their own and their child's advocate.
• Facilitate meetings and trainings.
• Report suspected child abuse and/or neglect following program procedures.

• Participate in outreach and recruitment activities and assist with enrollment of eligible children and families.
• Implements and monitors, with follow up, the agency's Attendance policy.
• Report and follow-up with home visits with any families demonstrating participation or attendance concerns.
• Maintains accurate records as required by the program while ensuring their confidentiality, i.e. child and family information.

• Collects, updates, and maintain children's health requirements within established timelines, and follow-up as needed, to ensure requirements are met.
• Conduct Health and Nutrition screenings at the time of enrollment.
• Initiate referrals based on screenings or parent concerns.
• Promote child and family wellness and the establishment of a medical and dental home.
• Provide health and nutrition education activities that are responsive to the ongoing and expressed needs of parents.
• Conduct hearing and vision screenings within the designated timelines.
• Collect child and family data and enter in Child Plus within the designated timelines.
• Monitor and update; prepare data reports for program use and the annual Program Information Report.
• Maintain accurate, objective, timely and confidential records documenting all family, staff, and agency contacts; disseminate appropriately.

Attend orientations, in-service training, educational programs, community meetings, conferences, and workshops as assigned.

Perform related duties as assigned.

Family Development Credential and High School Diploma OR Associate's Degree or higher in Social Work, Human Services, Family Services, Counseling, or a related field.

One (1) year of experience working with low-income families is required; experience in a preschool setting is preferred.

Position requires the ability to read, write, and speak fluently in Spanish.

• Clear criminal record with the Department of Justice following a fingerprint screening; (DOJ/FBI/CACI).
