Licensed Practical Nurse - Skilled Nursing & Rehab

Bethany Center for Rehabilitation and Healing seeks a compassionate Licensed Practical Nurse to deliver resident-centered care in our skilled nursing and rehab facility. The LPN will provide bedside nursing, administer medications and treatments, monitor vital signs, and document care in accordance with care plans and regulatory standards. Collaborating closely with RNs, therapists, physicians, and CNAs, the LPN supports short-term rehab and long-term care residents, educates patients and families, and helps maintain a safe, respectful, and healing environment that promotes dignity, comfort, and quality of life.

Responsibilities

• Provide direct bedside nursing care to short-term rehab and long-term care residents
• Administer medications and treatments per physician orders and facility policy
• Monitor, document, and report changes in resident condition to the interdisciplinary team
• Assist with admissions, discharges, and care plan implementation
• Accurately complete electronic health records and required documentation
• Collaborate with RNs, therapists, physicians, and CNAs to coordinate resident care
• Educate residents and families on care plans, medications, and health management
• Support infection control, safety, and quality assurance initiatives
• Participate in shift reports, team meetings, and in-service training
• Promote a resident-centered, respectful, and compassionate environment
