Olympic Health & Recovery Services is seeking a Mental Health Professional/Case Manager to join our Homeless Outreach Stabilization and Transition (HOST) team. This program follows the Assertive Community Treatment model and partners with mental health, substance use disorder, and medical staff to outreach to homeless individuals in the community, camps, or shelters that are living with severe behavioral health disorders. The Homeless Outreach Stabilization and Transition (HOST) Outreach Case Manager II provides support and case management services to eligible individuals who are living with serious substance use disorders or with co-occurring behavioral health conditions, are experiencing homelessness, and whose severity of behavioral health symptom acuity level creates barriers to accessing and receiving conventional behavioral health services and outreach models.
The ideal candidate for our Outreach Case Manager II is able to engage the most vulnerable individuals in a non-judgmental manner and harm reduction lens to provide support, case management, and appropriate participant driven treatment planning. The ideal candidate enjoys being outside engaging with individuals for a majority of their work day in non-traditional treatment settings and working within a multidisciplinary team.
Who are we? Thurston Mason Behavioral Health Administrative Organization (TMBH-ASO) and Olympic Health and Recovery Services (OHRS) is a quasi-governmental organization that is responsible for the network of behavioral health crisis services in Thurston and Mason counties, and is a licensed behavioral health agency with multiple programs assisting the most vulnerable population in our community through crisis services, jail services, hospital diversion, and housing.
This position reports to HOST Program Supervisor.

Salary Range: $38.10 - $46.01 hourly based on education, experience, and parity within the organization.

Overview of Major Responsibilities- Outreach and engage the most vulnerable individuals to assist with services, addressing behavioral and physical health needs.
- Provide support and case management to individuals to increase stability, assist to obtain housing, and transition individuals into long-term services for behavioral health conditions.
