We take pride in our staff and are seeking a Peer Specialist with a passion for supporting individuals experiencing mental health and/or substance use disorders who are at risk of incarceration.
The Peer Specialist position is part of the OHRS Forensic Projects for Assistance in Transition from Homelessness (FPATH) team. The role involves outreach and engagement to connect people to services who are at risk of referral for competency evaluations. FPATH provides voluntary, person‑centered, low‑barrier, assertive, and intensive case management for individuals identified as at risk for needing additional competency restoration services.
The position works to build relationships with eligible people in the community and help connect them with supports—including housing, basic needs, behavioral health, and health care—to reduce the likelihood of future criminal legal system involvement and to increase long‑term stability and wellness.
Salary Range: $24.78 – $32.22 per hour, based on education, experience, and parity within the organization.
Schedule:
Monday – Friday, 8:00 a.m. – 5:00 p.m.
TMBH‑ASO/OHRS provides comprehensive benefits for full‑time employees, including medical, dental, and vision plan options (many with no employee‑paid monthly premium depending on plan choice and dependent coverage options), life insurance, and disability insurance options. Additional tax‑deferred options include Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA). OHRS is an approved Public Student Loan Forgiveness employer. We pay the required DOH license fees for the position and offer annual professional development funds.
- Conduct outreach, in‑reach, and engagement with FPATH eligible referrals and participants.
- Share lived experience regarding criminal justice involvement, overcoming barriers, and model self‑help and wellness activities to empower participants on their recovery journey when appropriate.
- Provide supportive navigation, advocacy, information, and coaching during enrollment processes.
- Provide education and training to assist participants in acquiring skills that will maintain community stability and independence.
- Conduct regular community‑based street and/or encampment outreach in partnership with existing PATH programs, Homeless Continuum of Care organizations, or other local programs.
- Conduct regular in‑reach to local jails, hospitals, crisis facilities, court systems, and additional agencies.
- Build rapport with eligible referrals and complete enrollment into the FPATH program when participants consent to services.
- Provide case management services, including assessment of needs, development of a person‑centered goal/service plan, support participants in achieving goals, and connect to resources and services addressing social determinants of health.
- Provide voluntary, person‑centered, low‑barrier, assertive case management to participants at risk of needing competency restoration services.
- Develop relationships with community resources and provide coordination, support, and advocacy services to participants and families facing challenges such as homelessness, chronic mental or behavioral health issues, legal challenges, and substance abuse issues.
- Develop and maintain partnerships with local behavioral health, housing, and health care agencies to streamline referrals and enhance a continuum of care for eligible participants.
- Maintain EHR record‑keeping items.
