About the Position
Certified Occupational Therapy Assistant (COTA)     Skilled Nursing/Rehab     Laconia, NH (Weekdays)
Role overview  A   Certified   Occupational Therapy Assistant is needed for a weekday, full   time day   shift contract at a skilled nursing/post   acute rehab facility in Laconia, NH, working under the direction of an OT with adult and geriatric residents. This is a 13   week re   posted assignment (previously cancelled) focused on ADL independence, functional mobility, and safety in a SNF setting.

Skilled Nursing Facility (SNF) / geriatric post   acute and long   term care

Position type:
Contract, full-time, non   permanent

Contract length: 13 weeks (02/23/26     05/23/26)

Schedule:

8:00 AM     4:30 PM, 30   minute unpaid meal break

Days:
Monday   Friday

Guaranteed hours: 35 hours per week

Gross Blended Pay Rate: $33 - $42 p/h

Order reason:
Re   post     previous assignment cancelled

Key responsibilities      Implement OT treatment plans as directed by the supervising OT, focusing on self   care, ADLs, and functional activities.

Provide therapeutic activities targeting upper   extremity function, balance, coordination, and functional mobility.

Instruct residents in use of adaptive equipment and compensatory strategies to maximize independence and safety.

Observe, document, and communicate patient progress and changes in condition to the OT and interdisciplinary team.

Complete accurate, timely EMR documentation for all treatments and progress toward goals.

Required qualifications and skills      Active New Hampshire COTA license (or eligibility prior to start).

Graduate of an accredited Occupational Therapy Assistant program.

Current BLS/CPR for healthcare providers.

SNF or geriatric rehab experience preferred; at least one year of COTA experience ideal for a 13   week contract.
