About the Role
Do you have a passion for engaging with people and helping them on their journey to better health? Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
Location & Travel: Primarily traveling up to 100% of the time across a defined coverage area, inclusive of overnight hotel stays, car rentals and/or flights as required by business needs. Holding multiple state licenses will be required, as dictated by business needs.
Job Highlights: The visit lasts about 45 minutes, allowing time to connect one‑on‑one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will use an iPad and an intuitive clinical workflow for each evaluation. You are not required to prescribe medicine, order lab tests or alter people’s current treatment regimen.

- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Receive additional per‑completion incentive pay for all in‑home visits.
- Be flexible to travel locally and within licensed states, up to 100% of the time.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

Preferred Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Able to obtain and maintain multi‑state licenses in coverage area, as required.
- Able to cover the following primary locations: all counties within Maine and flex to other areas of business need.
- Maine Coverage Markets include:
Aroostook, Androscoggin, Kennebec, Knox, Lincoln, Sagadahoc, Waldo, Franklin, Oxford, Somerset, Hancock, Penobscot, Piscataquis, Washington, Cumberland and York.
The typical pay range for this role is $78,331.00 - $. This pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
Additional InformationCandidate must have primary residence in the state of Maine.
