• Provide direct nursing care to residents within the long‑term care setting according to care plans and facility policies
• Administer medications and treatments per physician orders; monitor for adverse effects and interactions
• Assess residents’ condition, document findings, and communicate changes to supervising RN or physician
• Assist with activities of daily living and support residents’ functional independence
• Monitor for changes in condition, respond to events, and elevate as needed per protocol
• Collaborate with the interdisciplinary team to ensure continuity of care and timely updates to the care plan

• Active LPN license in New Hampshire (or eligibility)
• Graduate of an approved practical nursing program
• Prior long‑term care or skilled nursing experience preferred
• CPR/BLS certification required or ability to obtain prior start date
• Strong communication and documentation abilities; ability to work effectively with a multidisciplinary team
• Able to work in a long‑term care setting and adhere to care plans and safety standards
