- Manage a defined caseload of patients within the Greater Lakes Region territory.
- Provide comprehensive nursing services and direct patient care in the community setting.
- Coordinate patient care, communicate with healthcare providers, and ensure care plans are effectively implemented.
- Maintain accurate documentation and adhere to company policies and regulatory requirements.
- Valid Nursing License (RN) in the State of New Hampshire.
- Minimum of two years acute care nursing experience.
- Current CPR BLS Certification (monthly renewal classes available and paid for by Interim Health Care).
- Reliable transportation to travel within the Greater Lakes Region.
- Home care experience is preferred but not required.
- Locally owned and operated company.
