Paid: $28.00 - $28.20 per hour ($1,008‑$1,015 weekly base pay with $0 weekly stipends).
Position:
Licensed Practical Nurse (LPN) – Contract – 9‑week assignment – 12 hours per day, 3 days per week – Minimum 36 hours guaranteed –

Under the direction of a Registered Nurse (RN), the LPN delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. The LPN operates within the scope of the State Nurse Practice Act, contributes to assessments and care planning, provides direct patient care, and supervises unlicensed staff.
Responsibilities- Patient evaluation:
Collect, report, and document objective and subjective data; observe conditions and report changes to RN. - Care planning:
Contribute to individual patient goals; assist in developing interventions; implement the plan of care; evaluate effectiveness; participate in review and revision of the plan. - Direct patient care:
Administer medications and treatments per physician orders; communicate pertinent data; document accurately; consult and seek guidance from RN; participate in utilization management or interdisciplinary care management meetings to facilitate discharge planning, communicating recommendations to physician, patient, and family; prepare patient/family for discharge including education. - Supervise unlicensed staff:
Assign duties per Act and policy; communicate patient information; monitor and evaluate duties; ensure tasks follow policies; use Point Click Care (PCC); perform drug counts; assist physician rounds; participate in shift‑to‑shift communication; collaborate with other departments; foster respectful, team‑oriented environment; attend mandated in‑service programs; assist orientation of new staff; promote safety culture; maintain confidentiality and PHI; stay beyond scheduled shift if needed;
perform other duties as requested.
- Comply with legal requirements, standards, policies and procedures including those within Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.
- Participate in orientation and training programs.
- Report concerns or non‑compliance promptly to supervisor, Compliance Liaison, or Compliance Officer via Integrity Hotline.
- Cooperate with monitoring, audit functions and investigations.
- Participate, when requested, in quality assurance and process improvement activities.
- Graduate of an approved practical or vocational nursing school.
- Currently licensed by the State Board of Nursing.
- Experience preferred in rehabilitative or geriatric nursing.
- Ability to read, write, speak and understand English.
- Provide verification of TST per state law; TSTs administered at work site if required.
- Maintain current BLS/CPR certification (required for all SNFs, Behavioral Centers, ALFs/ILFs as required by State).
- Comprehensive knowledge of nursing principles; ability to recognize symptoms and manage emergencies.
- Knowledge of medications, dosing, and expected outcomes.
- Ability to coordinate, delegate, and supervise unlicensed nursing functions.
- Ability to create patient‑centered environment.
- Basic computer skills.
