Registered Nurse

Laconia Rehabilitation Center in Laconia, NH is seeking a compassionate Registered Nurse to provide high-quality care to residents in our skilled nursing and rehab facility. The RN will perform assessments, administer medications and treatments, manage care plans, and monitor resident progress toward rehab and long-term care goals. Working in a resident-focused, team-oriented environment, you will collaborate with therapists, providers, and CNAs to ensure dignity, safety, and clinical excellence.

Responsibilities

• Deliver direct nursing care including assessments, treatments, and medication administration
• Develop, update, and implement individualized care plans for residents
• Monitor changes in resident condition and communicate with providers and families
• Document care accurately in EMR and maintain regulatory compliance
• Collaborate with therapy, social work, and other team members to support rehab goals
• Supervise and support LPNs and CNAs to ensure high-quality resident care
• Promote safety, infection control, and fall prevention practices
• Educate residents and families on care plans and health management
