• Provide direct nursing care to residents in a long-term care setting under supervision and per care plans
• Administer medications and document administration in resident records according to policy
• Monitor vital signs, observe resident status, and report changes to the supervising clinician
• Assist with activities of daily living and mobility, personal care, and safety measures
• Collaborate with the interdisciplinary team to update care plans and ensure continuity of care
• Maintain accurate charts, proper documentation, and infection control practices

• Current, active LPN license
• Ability to work night shifts (18:45–07:15) and meet guaranteed hours
• Minimum 1 year of experience in long-term care or skilled nursing preferred
• Graduation from an approved practical nursing program
• Basic documentation and charting competency; ability to follow established care plans
• Ability to work in a fast-paced long-term care setting and comply with facility policies
