This entry-level Field Registered Nurse position is designed to develop fundamental nursing skills within a home health setting while gaining an understanding of the company s operational goals and efficiencies. It serves as a foundational role for registered nurses aspiring to advance into case management.
This position works under a Case Manager Registered Nurse position.
Location:
Lamorinda Area.
- Providing hands-on patient care within RN scope (e.g., PICC lines, Wound Vacs & Care, Kangaroo & feeding Pumps, NG/G/J tubes management, staple & suture removal, drains such as JP and Biliary, Foley/suprapubic catheters with insertion/removal/troubleshooting, Pleur
X drainage, trach care, blood draws, ostomy management, SQ/IM injections, IV antibiotics/TPN administration, and compression wraps). - Provide direct patient care as defined in the state Nurse Practice Act.
- Implement a plan of care initiated and directed by the Case Management Registered Nurse.
- Provide accurate and timely documentation within 24–48 hours consistent with the plan of care.
- Assess and provide education to patient and family/caregivers pertinent to diagnosis and plan of care.
- Assist the RN in performing skilled procedures, including preparing equipment and teaching self-care techniques.
- Coordinate home health services and report identified needs to the Case Management RN or other disciplines as appropriate.
- Use equipment and supplies efficiently and participate in ongoing professional development.
- Participate in weekly Case Conferences and ensure patient privacy in medical records and care delivery.
- Maintain professionalism and productivity standards as directed; participate in Start of Care assessments and plans of care with the Case Manager as needed.
- Participate in on-call duties as required by weekend rotation and provide timely reports to the Case Manager and Provider.
- Document concisely to reflect patient needs, problems, capabilities, and responses to nursing interventions.
- Maintain open communication with the Case Management RN and other team members to coordinate patient care.
- One (1) year of recent nursing experience (experience requirement may be waived for internal candidates/new grad postings).
- One year of home health care experience is preferred.
- Experience with high-acuity patients is a plus.
- Experience working with an interdisciplinary team is highly desired.
- Graduate of an accredited school of nursing (Bachelor’s degree).
- Current CA Registered Nurse License (RN).
- Current Basic Life Support (BLS) Card.
- Valid CA Driver s License with acceptable driving record.
- All licenses and certifications must be current at time of hire and sustained throughout employment.
- Strong communication, critical thinking, organization, and computer literacy with the ability to document and problem-solve effectively.
- Maintain annual compliance training and certifications.
