Job SummaryThe Case Manager for the WCSO Detention Center will provide comprehensive case management services to inmates in the WCSO Detention Center. This position involves working closely with inmates, treatment providers, the court team, and community partners to support individuals in achieving recovery and re-entry into society. Key Responsibilities Case Management- Develop individualized case plans with inmates to address needs such as mental health treatment, substance use recovery, housing, employment, and transportation. Case plans are to be updated every thirty days (30) if the inmate remains incarcerated and upon re-entry to the detention center after release.
- Monitor and document inmates' progress toward case plan goals.
- Reassess as needed.
- Conduct regular check-ins with inmates to assess needs and provide support.
- Collaborate with treatment providers, probation officers, and community resources to ensure comprehensive care and services.
- Coordinate with the Program Director on any referrals for residential mental health treatment, residential substance abuse treatment, medical care, vocational training, and other needed services.
- Attend court sessions to provide updates on inmates’ progress and advocate for necessary resources.
- Prepare and maintain accurate records for use in court team meetings and compliance reviews.
- Respond to inmate crises, providing immediate support and appropriate referrals to the Medical Department or contacting the Program Director.
- Facilitate information to emergency services as needed.
- Empower inmates to achieve recovery goals through encouragement and education.
- Address barriers to participation, including transportation, housing instability, and lack of support networks. Assist with filling out housing applications and food stamp applications.
- Maintain detailed and timely records in the court’s case management system.
- Compile reports for internal and grant-related purposes.
- Actively participate in staff meetings, treatment team discussions, and program planning.
- Work closely with the Program Director, Clinical Director, and treatment providers to improve program outcomes.
Bachelor’s degree in social work, counseling, psychology, or a related field preferred.
ExperienceAt least two years of experience in case management, mental health, substance use treatment, or a related field. Experience in accountability courts or criminal justice settings is a plus. Experience in a jail setting is a plus.
CertificationsPreferred certifications include LS/CMI, ASAM, or training in evidence-based practices like Seeking Safety.
Skills- Strong organizational and communication skills.
- Ability to work collaboratively in a multidisciplinary team.
- Knowledge of trauma-informed care, mental health treatment, and substance use recovery.
- Proficiency in case management software and Microsoft Office.
- Ability to pass a background check and random drug screens
- Valid driver’s license and reliable transportation.
This role requires some flexibility, including occasional evening or weekend hours. The Case Manager will work primarily in an office setting but will also travel to community providers, inmates’ homes, and court sessions as needed.
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. While performing the duties of this job, the employee is usually exposed to a noise level that is low to moderate. This is a full-time position. Days and hours of work are normally Monday through Friday, 8:00 A.M. to 5:00 P.M. Occasional evening and weekend work may be required as job duties demand.
