This is a SNF / LTC assignment for an experienced or newly graduated Occupational Therapist / OT to join a growing team for a 13‑week contract.
ResponsibilitiesProvide direct patient care to assess medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
Establish and administer a treatment program with specific goals based on patient capacity and tolerance under physician direction.
Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families on appropriate occupational therapy methods.
RequirementsMust have graduated from an accredited school.
Current state license as an Occupational Therapist / OT or be in the process of obtaining licensure.
Current CPR certification.
Provide skilled occupational therapy services/interventions in accordance with physician orders.
Assess patient needs, plan for, evaluate and modify care to meet goals of occupational therapy interventions.
Collaborate with all disciplines to plan and evaluate team goals for each patient.
