Registered Nurse (RN) - Home Health PRN / Per Diem
Flexible Visits | Lafayette, IN | Pick Up When You Want

- What You'll Do
As a Home Health RN, you'll visit patients in their homes and help them safely manage their health conditions.
Responsibilities include:
* Completing routine skilled nursing visits
* Performing patient assessments
* Providing medication and disease-process education
* Monitoring wounds, vitals, and overall health status
* Communicating changes in condition to providers
* Coordinating care with therapists and care team members
* Documenting visits in the electronic medical record
* Following HIPAA and patient safety standards

- Training & Support
You won't be sent out alone. We provide:
* Structured onboarding
* Ride-along visits with experienced clinicians
* EMR documentation training
* Ongoing clinical support and case consultation

- Requirements
* Active, unrestricted RN license
* At least 1 year of nursing experience in any setting (hospital, SNF, clinic, rehab, etc.)
* Reliable transportation, valid driver's license, and auto insurance
* Ability to travel within the Lafayette coverage area
* Comfort working independently in the field
* Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Preferred (not required):
* Home Health or Hospice experience
* Home Care Home Base (HCHB) experience
* ASN or BSN

- Important Information
This is a PRN/per diem position and is not benefit-eligible (no health insurance, PTO, or 401k).
PRN nurses are scheduled based on availability and patient needs.
