A Home Health Registered Nurse develops and oversees nursing care plans that individually address each patient's needs. WHAT YOU CAN EXPECT

* Weekend Position
- Saturday and Sunday, 7am to 7pm and either Friday or Monday 7am to 7pm

* Minimum Start Pay $34.00/hour plus weekend shift differential for Saturday & Sunday shifts.

* Make initial nursing evaluation visit to communicate with physician and determine a plan of care.

* Practice nursing according to Franciscan Alliance and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.

* Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

* Collaborate with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience.

* Assess, collect, and analyze comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs, and plan of care.

* Develop an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously review, and revise the plan of care as patient condition warrants.

* Prioritize and implement the identified plan, coordinate care delivery, employ interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment.

* Evaluate the individualized plan of care and progress towards attainment of outcomes in collaboration with patient/family utilizing available data/resources.

* Communicate timely changes in patient's assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care, and maintain and coordinate communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery.

* Promote patient's independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; answering questions.

* Maintain a clean and safe work environment by handling and clearing instruments, equipment, and supplies, in accordance with infection-control policies and protocols.

* Ensure operation of equipment by completing preventive maintenance requirements, as per manufactures instruction or calling for repairs when required.

* Supervise clinical care provided by LPNs and Home Health Aides.

* Serve in an on-call rotation and provides patient care outside of regular business hours.

QUALIFICATIONS

* Associate's Degree
- Nursing - Required

* Bachelor's Degree
- Nursing - Preferred

* 1 year Acute Care Experience - Required

* 2 years Home Health Experience - Preferred

* 2 years Long Term Care Experience - Preferred

* Registered Nurse (RN)- State Licensing Board - Required

* Basic Life Support Program (BLS) - American Heart Association - Required

* Driver's License - Required

* Auto Insurance - Required
