Licensed Practical Nurse (LPN)
Bright Star Care is seeking a dedicated and clinically skilled Licensed Practical Nurse (LPN) to join our high-quality Home Health team. This full-time role offers stable hours, consistent weekly pay, and the opportunity to make meaningful, one-on-one connections with patients in the home setting.
As an LPN on our team, you will deliver essential skilled nursing care under the direction of a Registered Nurse, ensuring safe, compliant, patient-centered services aligned with state scope of practice and Bright Star Care standards.
Key Responsibilities – Licensed Practical Nurse (LPN)- Provide skilled nursing care in accordance with the Nurse Practice Act and Bright Star Care policies, procedures, and clinical protocols
- Implement the plan of care and treatment established by the Registered Nurse
- Perform all LPN-permitted skills, treatments, and procedures safely and competently
- Deliver services requiring substantial and specialized nursing skill
- Recognize changes in patient status and implement emergency measures promptly and appropriately
- Document care accurately, thoroughly, and in a timely manner following the nursing process and plan of care
- Communicate effectively with the Director of Nursing, Nurse Managers, physicians, and interdisciplinary team regarding changes in condition, progress, or patient needs
- Support medication administration, wound care, injections, disease monitoring, specimen collection, and other skilled tasks within the LPN scope
- Educate patients and families on care instructions, safety, and health management
- Promote patient comfort, autonomy, and high-quality outcomes in the home
- Active LPN license in good standing
- Minimum of 1 year of clinical nursing experience (home health preferred but not required)
- Strong clinical assessment and documentation skills
- Ability to work independently in a variety of home environments
- Excellent communication, professionalism, and reliability
- Competent in performing skilled nursing procedures within the LPN scope of practice
- Guaranteed 40 hours per week (full-time schedule)
