Licensed Practical Nurse

We are seeking a caring and experienced Licensed Practical Nurse (LPN) to provide direct nursing care in a long-term care facility.

• Administering medications, wound care, and performing skilled nursing procedures.
• Assisting residents with activities of daily living to ensure comfort, dignity, and safety.
• Supervising and directing nursing assistants and medication aides during shifts.
• Monitoring residents' physical and emotional status, notifying physicians of changes.
• Developing, implementing, and reviewing personalized care plans in collaboration with the interdisciplinary team.
• Completing accurate clinical documentation and coordinating admissions, transfers, and discharges.
• Participating in quality assurance, staff education, and regulatory activities.

• Active LPN/LVN license in the practice state.
• CPR/BLS certification preferred.
• At least one year of clinical nursing experience; long-term care experience is a plus.
• Strong clinical observation, medication, and documentation skills.
• Knowledge of federal and state long-term care regulations.
• Effective communication, independence, and sound clinical judgment.
• Experience with electronic health records and proficiency in Microsoft Office.
