Bright Star Care of Lafayette is hiring experienced Licensed Practical Nurses (LPNs) to provide skilled, one-on-one nursing care in patients’ homes. We offer guaranteed 40-hour schedules for full-time nurses, flexible part-time opportunities, weekly pay, supportive clinical leadership, and the chance to make a direct difference in patients’ health, comfort, and independence.
Qualifications- Active and unrestricted Indiana Licensed Practical Nurse license
- Graduate of an accredited practical nursing program
- At least one year of professional nursing experience required
- Home health, private duty, skilled nursing, or direct patient care experience preferred
- Strong clinical observation and documentation skills
- Demonstrated competency performing nursing treatments within the LPN scope of practice
- Ability to work independently in a variety of home environments
- Excellent communication, reliability, and time-management skills
- Comfortable using electronic health record technology
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to satisfy all required employment and health screenings
- Provide skilled nursing care in patients’ homes according to physician orders and the established plan of care
- Implement RN-developed plans of care accurately and consistently
- Perform nursing procedures, treatments, and other clinical services permitted within the LPN scope of practice
- Administer medications and injections safely and accurately
- Perform wound care, specimen collection, disease monitoring, and other ordered treatments
- Monitor vital signs, symptoms, and overall patient condition
- Recognize changes in condition and promptly report or escalate concerns to the supervising RN or Director of Nursing
- Complete accurate, thorough, and timely electronic documentation
- Communicate with the Director of Nursing, Nurse Managers, physicians, and other members of the care team
- Reinforce patient and family education regarding medications, treatments, safety, and health management
- Follow infection-control practices, clinical protocols, company policies, and the Indiana Nurse Practice Act
- Promote patient comfort, safety, dignity, independence, and optimal outcomes
