Nurse Practitioner - PRN

Join a national in-home clinical network as a Nurse Practitioner, providing comprehensive health assessments in members' homes or facilities. Responsibilities include conducting adult/geriatric assessments, reviewing medical histories, medication adherence, and social factors, and collaborating with healthcare teams and primary care physicians to improve care quality and address care gaps. You will administer tests, document findings, and offer health education and referrals as needed.

• Perform assessments up to one hour, capturing detailed health and social information
• Coordinate with healthcare providers and caregivers to ensure comprehensive care
• Provide services in members' residences, including skilled nursing facilities
• Complete documentation per guidelines and protocols
• Support care management and follow-up activities

• Master’s Degree or equivalent with NP licensure and prescriptive authority
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care
• Current BLS, ACLS, or CPR certification
• Strong communication skills, computer proficiency, and a valid driver’s license
• Preferred: 1+ year NP experience; new grads encouraged
